Preparing For Your Stay

When visiting Hillside Rehabilitation Hospital, patients should bring the following:

• All current health insurance information, including your insurance card

• Physician orders and prescriptions for any tests you will receive

• Referrals from your primary care physician (if your insurer requires them)

• A payment method for deductibles and co-payments (financing is also available)

• Your Advance Directive  if you have one

• A photo ID

• And if you're having a procedure, remember to have a responsible individual to drive you home

Please arrive a few minutes before your appointment.

Visitors to Hillside Rehabilitation Hospital may park, free of charge, in our convenient main parking lot, just a few yards away from our front entrance.
